Newbie shooter, one hole group, oops!

I have a rather tight indoor range in my machine shop, sort of like threading a needle. My brother stopped out and fired five rounds into 'one hole' but I've seen one hole groups before and this wasn't one of them. I told him something was amiss, but he said until he found out otherwise, he was going to believe he shot a perfect five shot group. A couple of days later I noticed light coming from keyhole shaped holes in my drywall and investigated to find a new collet rack that wasn't quite right. Here is a picture;

Explains why we have to use a moving backer system in short range Group Matches.

Next time he comes in, hang a 100 yard score target and tell him to hit 5 X's.
