Not me, but I know the person well.
I've seen blanked primers, but not one this bad before - a blown 223 case expanded tightly into the Rem 700 bolt head.
I got the bolt open with a mallet, but the case couldn't be removed from the bolt. See the hole blown out by the case head - much gas released, his shooting glasses were blown off, but there were no injuries.
Also he was at the range alone. What would he have done if it had been more serious?
Lesson: Never shoot free, "gimme" hand loads; always wear shooting glasses; and Remington's "3 rings of steel" works.
If it was cut for a Sako extractor it could have been much worse. Let's be careful out there!
